正面，太上咒曰：天圆地方、六律九章、符神到处、万鬼灭亡、急急如律令奉，敕摄此符神灵。 咒文由方框围合，寓意祈福驱邪。
背面，穿右为真武披发仗剑于龟蛇合体玄武之上，穿上为符咒，左左右俩侍卫。
